Hypoxic therapy and autoantibodies after gynecological surgery: a 2005 study. Observation of 115 patients, immune measures, and postoperative recovery.

A detailed editorial account of a 2005 clinical publication. The authors compared groups of unequal size and predominantly immunological measures. The observation does not establish a causal improvement in fertility. The full text, graphs, and bibliography are available in the original PDF.

Publication at a glance

  • The study included 115 patients: 93 in the intervention group and 22 in the comparison group.
  • Regulatory autoantibodies after gynecological surgery were studied.
  • Changes in immune markers do not establish improved fertility or pregnancy outcomes.

Hypoxic therapy and autoantibodies: the study’s aim

I. Z. Lavrinenkova and colleagues studied recovery after gynecological surgery. The main question concerned natural regulatory autoantibodies. The authors assessed their changes after intermittent normobaric hypoxic therapy.

The work concerns postoperative rehabilitation of women of reproductive age. It is not a study of OXYTERRA RS. The authors used other breathing systems, including the Bio-nova-204 hypoxic generator.

Participants and comparison groups

The study included 115 patients aged 17–36 years. Mean age was 26±0.5 years. Operations included myomectomy, salpingectomy, oophorectomy, and ovarian resection.

The intervention group included 93 women who received hypoxic therapy in the postoperative period. The comparison group consisted of 22 patients receiving conventional recovery measures. The authors list antibacterial and other therapy among these.

The total follow-up period was one year. The description does not establish that the groups were equivalent in all prognostic factors. Numerical differences between them therefore require cautious interpretation.

Measures assessed

The examination program included routine clinical laboratory tests, blood biochemistry, a coagulation profile, ultrasound, and hormonal measures. For IgG-class regulatory autoantibodies, the authors used the ELI-P test. It reflected serum antibodies to several regulatory proteins.

Immunological examinations took place two weeks after surgery, after rehabilitation therapy, and after three months. Tubal patency was additionally assessed in some women. Reproductive events were tracked for a year.

Breathing course and tolerance

The work describes mixtures containing 10–12% oxygen at normal atmospheric pressure. The authors report intermittent and continuous regimens. A physician specializing in physical medicine selected the parameters and total exposure according to the patient’s condition.

At the start of the first session, some women experienced air hunger, dizziness, and palpitations. The authors also noted increased blood pressure. Symptoms resolved after removal of the mask and a return to normal air. A claim of a complete absence of adverse reactions would therefore be inconsistent with the study description.

Changes in immune measures

Two weeks after surgery, the authors identified hyporeactivity in 96 of 115 patients. Hyperreactivity was noted in 14 and normal reactivity in five. The authors describe the baseline profiles of the two groups as similar.

After the course, hyporeactivity persisted in 24 of 93 women in the intervention group. Hyperreactivity was identified in 57 and normal reactivity in 12. After three months, the corresponding numbers were 21, 41, and 31.

In the comparison group, hyporeactivity persisted after rehabilitation therapy in 18 of 22 participants. Thus, the work describes differing changes in the laboratory profile. This is not equivalent to evidence of improvement in all clinical outcomes.

Clinical observations and significance of the work

The postoperative period was uncomplicated in both groups. The authors report faster improvement in well-being in the intervention group. They describe reduced pain, changes in psychological and emotional state, and recovery of work capacity.

During the year, 11 women in the intervention group and one in the comparison group became pregnant. However, the groups differed in size, and not all participants were planning pregnancy. These observations do not establish that hypoxic therapy itself increased the likelihood of conception.

The work’s main contribution is its description of immunological changes during postoperative rehabilitation. Study design and the participants’ baseline condition are important for assessing their clinical significance. The authors’ complete results and graphs are retained in the original article.
